We strongly recommend that you also notify our Histology department via EMAIL once the specimen has been shipped out.

Shipping

1. Label all containers with patient’s name, specimen source, and a second patient identifier (e.g., MRN or date of birth).

2. Label all submitted slides with patient’s name (and all info listed above if not within a fully labeled container).

3. Prepare slides/container prior to collection, ensure that collection vials are NOT expired, and double-check after collection. 

Specimen Labeling

Requisition Forms - Required Information

1) Name of Requesting Physician.

2) Full Name of patient; clearly and correctly spelled.

3) Patient’s SSN or MRN (strongly requested).

4) Patient’s sex and date of birth.

5) Date of specimen collection.

6) Patient’s address

7) Attach copy of insurance card (front and back).

8) ICD-10 code(s)

9) Exact anatomic source of specimen and test(s) requested

10) Pertinent patient history and clinical information.

Cytology & Surgical

Order form for GYN Cytology and any additional Surgical work to be performed                                    

Surgical Pathology

Order form for Surgical Cases; Renal, Non-GYN Cytology, and Surgical Pathology orders.                                                                            

Prenatal Requisition Form

Order form for Non-Invasive Prenatal Testing (NIPT) and Products of Conception (POC) testing.
